Our Commitment to Patient Safety
At Psicon, ensuring safe, compassionate and person-centred care is at the heart of everything we do. We recognise that many of our clients come to us following long waits or complex experiences, and with that comes a responsibility to continually improve the safety and quality of the care we provide.
Our Patient Safety Incident Response Plan (PSIRP) explains how we respond to patient safety incidents (PSIs), how we learn from them, and how that learning is used to improve our services.
It reflects our commitment to openness, continuous learning and delivering the highest standards of care for every person who uses our services.
Our Approach
We have adopted the principles of the NHS Patient Safety Incident Response Framework (PSIRF) across all of our services, including both NHS and privately funded care. Our approach is built on the understanding that patient safety is influenced by many interacting factors.
Learning Rather Than Blame
Rather than focusing on individual blame, we seek to understand how systems, processes and environments contribute to incidents so that lasting improvements can be made. We are committed to fostering a just culture where staff, patients and families feel safe to raise concerns, report incidents and share their experiences.
We believe that the best learning comes from honest conversations, compassionate engagement and understanding why something happened rather than who was responsible. This creates the psychological safety needed for continuous improvement and helps us prevent similar incidents from happening again.
Our approach is underpinned by our Duty of Candour responsibilities and includes providing support, sharing information and keeping those affected informed throughout the review process.
